Founded in 1949, Mount Sinai Medical Center is the largest private independent not-for-profit teaching hospital in South Florida. Mount Sinai Hospital's Health Records department Monday to Friday between 8:30 a.m. and 4:30 p.m. Please bring your OHIP card. We are currently working on a phased roll-out that will bring MyChart to patients across Mount Sinai Hospital. As MyChart registration becomes available in other areas, we will update this webpage.

CLINICAL INFORMATION. Look up selected clinical information - lab results, medical imaging reports, and clinical notes (including progress reports, discharge summaries, operative reports, etc.) are all available online.

MyChart Activation Code. Enter your Activation Code as it appears on your enrollment letter (your code is not case sensitive). You will not need to use this code after you complete the signup process.
